Sports Physicals

A sports physical is needed for most high school sports/activities. Sports physicals are good for 3 years, and must be valid through the entire season (through the state meet) in order to be cleared. Most students get their sports physicals before their 7th and 10th grade year. If a sports physical is not current, you will NOT allow you to complete the registration process. Completed sports physical forms may be submitted by:

• dropping off in the Activities Office
• faxing to: 651-982-8412
